Traditionelt er denne variation ofte blevet h\u00e5ndteret reaktivt: Afdelingen tilpasser sig, n\u00e5r presset opst\u00e5r. Den tilgang er forst\u00e5elig, men den har omkostninger.<\/p><p>Litteraturen peger p\u00e5, at belastningen i akutarbejde ikke alene afh\u00e6nger af arbejdsm\u00e6ngden, men ogs\u00e5 af graden af forudsigelighed, indflydelse p\u00e5 arbejdstilrettel\u00e6ggelsen og oplevelsen af organisatorisk st\u00f8tte. Manglende overblik over kommende belastning kan derfor ikke kun p\u00e5virke driften, men ogs\u00e5 medarbejdernes trivsel, oplevede kvalitet i arbejdet og lyst til at blive i faget. [1\u20133]<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-e314853 e-con-full e-flex e-con e-child\" data-id=\"e314853\" data-element_type=\"container\" data-e-type=\"container\">\n\t\t\t\t<div class=\"elementor-element elementor-element-7319d6c elementor-widget elementor-widget-heading\" data-id=\"7319d6c\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Uforudsigelighed som belastningsfaktor<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-6f2100a elementor-widget elementor-widget-text-editor\" data-id=\"6f2100a\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p style=\"font-weight: 400;\">En gennemg\u00e5ende pointe i arbejdsmilj\u00f8litteraturen er, at belastning forst\u00e6rkes, n\u00e5r medarbejdere har begr\u00e6nset kontrol over arbejdets organisering. I et multicenterstudie fra schweiziske psykiatriske hospitaler fandt Gehri et al. at lav oplevet kontrol over vagtplanen, begr\u00e6nset st\u00f8tte fra ledelsen i planl\u00e6gningen og overarbejde var associeret med emotionel udmattelse og intention om at forlade arbejdspladsen. Studiet er ikke gennemf\u00f8rt i akutmodtagelser, men mekanismen er sandsynligvis relevant ogs\u00e5 her: uforudsigelighed og kortsigtede \u00e6ndringer i arbejdstilrettel\u00e6ggelsen slider p\u00e5 medarbejderne. [1]<\/p><p style=\"font-weight: 400;\">Tilsvarende viste Ghaseminejad et al. i en kvalitativ unders\u00f8gelse blandt akutmedicinere og internmedicinere, at tunge arbejdsbyrder, planl\u00e6gningsforhold, hyppige afbrydelser, tv\u00e6rg\u00e5ende konflikter og oplevelsen af ikke at blive v\u00e6rdsat af ledelsen var centrale organisatoriske forhold bag burnout. Blandt akutmedicinere fremh\u00e6vedes desuden vold i arbejdsmilj\u00f8et og vurdering af patienter i venteomr\u00e5der som s\u00e6rlige belastninger. I rapporten \u201dHvorfor stopper sygeplejersker i det offentlige sundhedsv\u00e6sen?\u201d beskriver tidligere ansatte sygeplejersker blandt andet arbejdspres, utilstr\u00e6kkelige rammer og manglende mulighed for at udf\u00f8re arbejdet p\u00e5 en fagligt forsvarlig m\u00e5de som v\u00e6sentlige \u00e5rsager til at forlade det offentlige sundhedsv\u00e6sen. Rapporten er ikke afgr\u00e6nset til akutafdelinger alene, men den underst\u00f8tter, at organisatoriske forhold spiller en central rolle for fastholdelse. [3]<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-1c0bdea e-con-full e-flex e-con e-child\" data-id=\"1c0bdea\" data-element_type=\"container\" data-e-type=\"container\">\n\t\t\t\t<div class=\"elementor-element elementor-element-b630695 elementor-widget elementor-widget-heading\" data-id=\"b630695\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Konsekvenser af reaktiv drift\n<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-563b89f elementor-widget elementor-widget-text-editor\" data-id=\"563b89f\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p style=\"font-weight: 400;\">N\u00e5r bemanding og kapacitet hovedsageligt justeres, efter at presset er opst\u00e5et, f\u00e5r det ofte afledte konsekvenser. Et nyere studie viste, at lav bemanding, overarbejde p\u00e5 seneste vagt, flere arbejdsdage i tr\u00e6k og l\u00e6ngere vagter var associeret med h\u00f8jere oplevet arbejdsbelastning m\u00e5lt med NASA-TLX scoren. Studiet fandt derimod, at quick returns (alts\u00e5 at m\u00f8de igen p\u00e5 arbejde kort efter at have f\u00e5et fri) var sj\u00e6ldne i det unders\u00f8gte materiale, s\u00e5 netop den pointe b\u00f8r ikke overdrives. [4]<\/p><p style=\"font-weight: 400;\">Et andet studie viste, at sygeplejerskevikarer blev anvendt p\u00e5 12,2 % af vagterne, men at denne type indsats kun i begr\u00e6nset omfang lukkede hullerne efter afvigelser i planlagt bemanding. Over halvdelen af frav\u00e6rssituationerne blev ikke adresseret med midlertidigt personale. Resultaterne peger derfor p\u00e5, at akut og midlertidig opnormering ikke i sig selv er en robust l\u00f8sning p\u00e5 mismatch mellem behov og kapacitet. [5]<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-f3724f9 elementor-widget elementor-widget-heading\" data-id=\"f3724f9\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Betydningen af bedre kendskab til eftersp\u00f8rgslen\n<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-ee1f333 elementor-widget elementor-widget-text-editor\" data-id=\"ee1f333\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p style=\"font-weight: 400;\">Hvis belastningen i akutmodtagelsen i nogen grad kan forudsiges, \u00e6ndrer det ledelsesopgaven. Det giver mulighed for at flytte planl\u00e6gning fra mavefornemmelse til datast\u00f8ttet beslutningstagning. Studier af bemanding og arbejdsbelastning peger samlet p\u00e5, at bedre tilpasning mellem behov og ressourcer kan mindske oplevet belastning, reducere overarbejde og styrke oplevelsen af kontrol. N\u00e5r bemandingen systematisk ligger for lavt i spidsbelastninger, stiger risikoen for overarbejde, akut indkald, brug af midlertidigt personale og tab af erfarne medarbejdere. N\u00e5r den ligger for h\u00f8jt i lavbelastningsperioder, bindes ressourcer uden tilsvarende v\u00e6rdi.<\/p><p style=\"font-weight: 400;\">Litteraturen giver dermed st\u00f8tte til en mere n\u00f8gtern konklusion: organisatorisk forudsigelighed og bedre planl\u00e6gningsgrundlag er ikke blot sp\u00f8rgsm\u00e5l om effektivitet, men ogs\u00e5 om arbejdsmilj\u00f8 og fastholdelse. Tilg\u00e6ngelig litteratur peger p\u00e5, at lav kontrol over arbejdstilrettel\u00e6ggelsen, overarbejde, afbrydelser og utilstr\u00e6kkelig organisatorisk st\u00f8tte er forbundet med udmattelse, \u00f8get belastning og sv\u00e6kket fastholdelse. [1\u20134]<\/p><p style=\"font-weight: 400;\">Det taler for, at arbejdet med bedre forudsigelse af patienttilstr\u00f8mning og mere pr\u00e6cis kapacitetsplanl\u00e6gning b\u00f8r forst\u00e5s som en organisatorisk intervention, ikke kun som et teknisk styringsv\u00e6rkt\u00f8j. Potentialet ligger b\u00e5de i bedre ressourceudnyttelse og i et mere b\u00e6redygtigt arbejdsmilj\u00f8 for de medarbejdere, der b\u00e6rer den akutte drift.
